Use cases for this template

Falcon Youth Gear's Summer Kickoff Shoot

The Challenge

Riverstone Creative landed a fast-turn commercial for Falcon Youth Gear featuring youth athletes, but the team had only four days to schedule a park shoot with 18 minors, collect parent consents, and align usage rights for streaming, broadcast, and social media, all while coordinating with a city parks permit.

The Solution

They used Proposal Kit to assemble a tailored model release form packet and to create supporting documents-an on-set parent info sheet, a photo/video usage overview, and a clearance workflow brief-while the AI Writer drafted a shoot plan and a postproduction rights summary; line-item quoting helped the producer scope budget for a clearance coordinator and translation services.

The Implementation

The production manager assigned job numbers that matched each release and shot list, handed parents the packets at check-in, and reviewed the name-use clause and the waiver of inspection; the team archived signed forms in the project binder and used the AI Writer to produce a follow-up memo for brand partners confirming the breadth of rights and the arbitration venue specified in the release.

The Outcome

Falcon Youth Gear launched on schedule with clean chain-of-rights documentation, the distributor accepted the package without revisions, and the brand reused clips for a mid-season social push without re-clearance delays.

BrightWave Health's Pediatric Wellness PSA

The Challenge

BrightWave Health planned a bilingual public-service campaign in clinics across two counties, filming minors in waiting rooms and exam hallways; they needed consistent consent practices, dual-parent signatures where possible, and a simple way to explain the no-approval and editing provisions to families.

The Solution

Using Proposal Kit, the team assembled state-specific model release form packets and produced supporting materials: a parent FAQ, a plain-language privacy and publicity explainer, and a clinic staff briefing; the AI Writer created a shoot-day script for volunteers and a distribution plan outline, while line-item quoting earmarked budget for interpreters and a dedicated consent lead.

The Implementation

Clinic coordinators held short parent sessions using the FAQ to describe recording of actions and speech and the possibility of name use, then collected signatures and logged job numbers tied to each location; the producer filed the paperwork with project records and sent partners an AI Writer-generated summary of the governing law and arbitration options in the release.

The Outcome

The PSA aired on local TV and streaming channels with zero rights queries, the insurer accepted the E&O application on first pass, and BrightWave repurposed footage for a grant report without additional legal reviews.

EduSpark's Student Beta Launch Video

The Challenge

EdTech startup EduSpark wanted a launch video featuring middle school testers using its app, but school administrators required clear parental consent, a description of potential edits like composites and voiceover, and documentation aligning with district policies.

The Solution

EduSpark used Proposal Kit to assemble a comprehensive model release form set and to create supplemental documents-an administrator briefing note, a parent letter, and a risk and usage overview-while the AI Writer produced a filming schedule, an asset-naming guide tied to job numbers, and a short report for investors; line-item quoting clarified costs for school liaisons and after-hours shoots.

The Implementation

On filming day, the team verified guardian signatures, explained the waiver of inspection and broad media rights, and labeled each clip with the matching job reference; after wrap, they archived the releases with edit decision lists and sent the district the AI Writer-drafted summary of rights scope and venue language.

The Outcome

The video cleared app store and platform reviews without delay, the district approved continued filming for future updates, and EduSpark leveraged the footage in a successful fundraising deck supported by the investor report.

Abstract

This contract is a model release form tailored for situations where a young person appears in photos, video, or audio. It gives the photographer or production company broad, irrevocable permission to use the minor's image, likeness, and recorded actions or speech in any medium, now or in the future. It also allows editing and creative changes, including airbrushing, blurring, scenes, or added dialogue.

The company may choose whether to use the person's name. The signer waives any right to review or approve the final materials.

The release includes a comprehensive waiver of claims. It releases the company and its agents from liability tied to privacy, publicity, and defamation claims, as well as claims related to lost wages, employment issues, residuals, license fees, taxes, or compensation. In short, once signed, the company can publish and republish the materials without owing further payment. The contract states it is the entire agreement and requires any changes to be in writing and signed by both parties.

The agreement identifies governing law and venue by state and county, which the parties will fill in. It binds the photographer's successors and assigns, with limits on assigning obligations without the signer's written consent. The document also includes a job or contract reference number to support production tracking. It asks for the signer's full legal name and date, and it anticipates providing a Social Security Number, which some organizations collect for internal records.

If the participant is under 18, a parent or legal guardian must sign. The contract urges obtaining signatures from both parents when possible to prevent disputes later. The parent or guardian agrees to defend and hold the beneficiaries of the release harmless from any future claims by the minor. Disputes must first be submitted in writing to the company, which then has the option to require binding arbitration under American Arbitration Association rules in the named county and state.

Use cases include commercial photography, film and TV productions, streaming content, social media campaigns, branded advertising, and educational or editorial projects where minors appear on camera. This model release form helps teams clear rights before distribution.

Proposal Kit can streamline building and managing documents like this. Its document assembly, automated line-item quoting, AI Writer for supporting materials, and extensive template library help businesses produce consistent, professional releases and related paperwork with ease.

Beyond granting permission to use a minor's likeness, this model release form shapes how creative teams plan, clear, and distribute content. Its broad grant of rights, including permission to edit, composite, and fictionalize, supports downstream needs such as network delivery, streaming platform acceptance, and errors-and-omissions insurance. Distributors and brand partners often require proof that talent rights were cleared without limitations; this document provides that chain-of-title certainty, reducing takedown risk and production delays.

Two operational points stand out. First, the notice that actions, speech, and appearances may be recorded even when devices are not obvious helps documentary, reality, event, and behind-the-scenes shoots proceed without constant re-consent. Second, the waiver of inspection and approval protects editorial discretion, which is critical when cutting trailers, short social clips, and composite images.

Because minors cannot contract on their own, the form's parent or guardian consent, plus the recommendation to get both parents to sign, mitigates disputes after publication or when the child turns 18. The parents' agreement to defend and hold harmless the beneficiaries further stabilizes long-term usage.

The agreement's governing law, venue, and arbitration provisions add predictability. Having the option to require binding arbitration can reduce cost and schedule impact if disagreements arise. The identification fields (job number, names, dates, and optional Social Security Number) support recordkeeping, payroll, or accounting alignment when needed, and audit trails. Treat any personal information with appropriate security and limit access to those who need it.

Typical use cases include youth sports marketing, school and camp photography, nonprofit fundraising campaigns, healthcare and education awareness videos, product demos featuring teen testers, retail in-store displays, and sponsored social content. Teams benefit by securing signed releases early in pre-production, confirming on set, and archiving the forms with project files so future reuse, like compilations or anniversary retrospectives, remains clear.

Proposal Kit can help teams assemble and manage consistent paperwork around these workflows. Its document assembly speeds creation of tailored releases and related production documents; automated line-item quoting helps prepare budgets and estimates that accompany shoots; the AI Writer can generate supporting materials such as instructions, checklists, and cover notes; and the extensive template library makes it easier to standardize your model release form across projects.

A release for minors also influences distribution strategy, content governance, and brand risk management in ways that go beyond legal clearance. Because the grant of rights reaches any medium now or hereafter known, teams can plan confidently for future uses such as streaming platform remasters, augmented reality experiences, or internal training and marketing assets derived from the same shoot. The permission to copyright the materials supports internal asset management and licensing to partners, which helps advertising and entertainment companies maintain a clean chain of rights across campaigns, sizzle reels, and compilations.

Production operations benefit from clear, repeatable workflows tied to each shoot. Create a linkage between the job or contract reference number on the model release form and your shot lists, timecode logs, and asset IDs so producers can prove clearance for a specific clip or still on demand. On set, many teams use check-in stations and color-coded wristbands or badges to confirm that parent or guardian consent is on file before the minor approaches the camera. After wrap, archive signed releases alongside selects and edit decision lists so reuse months later is frictionless.

The clause allowing use with or without a name impacts metadata and credits planning. If the company chooses not to publicize the name, ensure your internal databases can use a project ID or alias while still connecting to the correct rights paperwork. When collecting sensitive information such as a Social Security Number for internal accounting, restrict access and align your retention schedule with your broader records policy to reduce exposure.

From a people perspective, consider communications that set expectations for families. Even though the agreement waives inspection, many brands provide a brief overview of where content might appear and how long projects typically run. Courtesy updates help maintain goodwill during long campaigns and reduce inbound questions to production staff.

Proposal Kit supports these processes by helping teams assemble consistent, project-specific paperwork quickly. Use its document assembly to insert the correct state, county, and arbitration options, and to create variations tailored to schools, nonprofits, or commercial shoots. Automated line-item quoting can allocate budget for clearance staff and post-production admin. The AI Writer can write crew checklists, cover notes for parents, and archive instructions, while the extensive template library helps standardize your release package across projects with ease.
