The Title Page is usually a more graphical cover for the proposal which follows. It describes very briefly the project and includes basic contact information and optionally your company logo.

Note: The Proposal Number can be your own internal proposal identifier. If you are responding to an RFP you should change it to the RFP number and change the field from "Proposal Number" to "RFP Number".
